About the Role
We are seeking a skilled and compassionate Social Worker to join our Adults Social Care team, supporting older adults and those with physical disabilities. You will work collaboratively with residents, families, carers, and partner organisations to promote wellbeing and independence. Your responsibilities will include carrying out complex assessments, risk assessments, and safeguarding enquiries, applying strengths-based approaches and evidence-based practice while upholding professional standards. You may also conduct Mental Capacity Act assessments, ensuring decisions are made with and for the individual wherever possible.

Requirements

Experience
- Relevant experience working effectively as a practising Social Worker
- Experience working with vulnerable adults
- Experience in operating within a comprehensive assessment framework including self-assessment
- Experience preparing clear and accurate correspondence, reports, and other written documentation
- Proven record of delivering a customer-focused service to demanding targets and objectives
- Experience actively participating in cross-functional networks and local groups

Qualifications
- Relevant Social Work degree qualification
