About the Role
We are seeking three experienced Social Care Practitioners to undertake planned reviews for adults receiving support from the London Borough of Bromley. Working as part of the Coordination and Review Team, you will carry out assessments and reviews in accordance with the Care Act 2014. This includes reviewing the needs and support arrangements of adults residing in care homes, Shared Lives schemes, Extra Care Housing, or those receiving domiciliary care services through Direct Payments or commissioned care packages, including individuals placed in residential care settings.
The role requires travel within and outside the borough to visit and review individuals in their current placements.
Working pattern is hybrid, with a minimum of 2 days per week office attendance.

Experience
- Experience working in Care Management settings
- Recent experience in adult social care, ideally with older people and/or housing-based support services
- Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding duties
- Experience completing assessments and reviews for people receiving Bromley funded care packages in their own homes, Extra Care, supported housing, residential and nursing placements, or similar settings
- Confidence working autonomously and managing a complex caseload

- Ability to work collaboratively with care providers, housing and health professionals to ensure continuity of care
- Skilled in identifying risks associated with changes to care provision and implementing appropriate risk management and safeguarding measures
- Competent in undertaking Mental Capacity Assessments and contributing to Best Interest decision-making
- Committed to ensuring individuals, families, and advocates are fully informed and supported
- Strong organisational skills to maintain accurate, timely, and defensible case recording in line with statutory and organisational requirements

Desirable
- Experience supporting service transitions, hospital discharges, or provider changes
- Experience of partnership working with housing providers
