About the Role
Guy’s and St Thomas’ NHS Foundation Trust is one of the UK’s busiest and most successful NHS trusts, providing a wide range of hospital and community services across south London and specialist care for patients from further afield. The @Home team, an accelerator site for Urgent Community Response across South East London, offers a Hospital at Home service covering Lambeth and Southwark. This service delivers intensive multidisciplinary acute clinical care at home that would otherwise be provided in hospital.
The Administrator role supports the @Home team by managing referrals received via the Unified Point of Access, providing clinical and administrative support to a large multi-disciplinary team, managing inboxes, and handling patient or clinical queries by phone. The service operates 7 days a week from 08:00 to 23:00, caring for patients ranging from those needing a single episode of care to those with complex, long-term conditions requiring periodic short-term interventions to maintain quality of life at home.
The service accepts referrals from GPs, London Ambulance Service, Guy’s and St Thomas’ Hospitals, King’s College Hospital, St George’s Hospital, and other community services. The @Home team focuses on same day or next day responses to support early hospital discharge and hospital admission avoidance through close collaboration with multiple healthcare and social care partners.
