We are seeking an experienced, dynamic, and strategic Project Manager to join the Integrated Care Transformation Team. This role will lead key projects across South-East London, driving system change using Population Health Management principles. You will manage complex programmes, collaborate with clinicians, managers, and senior executives, and ensure delivery of high-quality patient-centred care through cross-organisational transformation.

The Integrated Care Transformation Team works closely with partners across the South-East London Integrated Care System to deliver transformation and quality improvement programmes focused on integrated neighbourhood teams, value-based care, efficient transfers of care, and embedding population health principles.

This is a fantastic opportunity for a motivated individual passionate about transformation and improvement in health and care outcomes.

Requirements

Experience
- Proven track record managing successful projects and operational improvements, including business case preparation, service commissioning, contract negotiation, and monitoring outcomes.
- Experience managing complex services and change programmes in multi-stakeholder environments.
- Demonstrated ability to work with diverse partners to deliver person-centred change.
- Experience managing complex budgets from multiple income sources.
- Desirable: Knowledge of the evolving health and care environment, strategy, and policy agendas.

Qualifications
- Evidence of a Masters or other postgraduate qualification, or equivalent experience.
- Formal project management qualification (PRINCE2 Practitioner level or equivalent).
- Evidence of recent Continuing Professional Development.

- Extensive knowledge of project and operations management methodologies.
- Excellent change and project management skills with a strong track record.
- Highly effective people management and leadership skills.
- Advanced analytical and judgement skills, including interpreting complex statistical and numerical data.
- Excellent interpersonal, communication (written, oral, presentation, facilitation), networking, and negotiation skills.
- Strong organisational skills with the ability to prioritise, meet deadlines, and delegate effectively.
- Innovative problem-solving abilities.
