About the Role
Guy’s and St Thomas’ NHS Foundation Trust is one of the UK’s busiest and most successful NHS trusts, providing a wide range of hospital and community services across south London and specialist care for patients from further afield. We pride ourselves on delivering safe, high quality care with a strong focus on patient experience.

The RXF: SELCA Patient Pathway Navigator role requires an enthusiastic, dedicated and adaptable individual with excellent attention to detail. The successful candidate will demonstrate strong prioritisation and problem-solving skills, a collaborative approach, and the ability to work well under pressure and as part of a team. This role is key to ensuring the smooth and efficient management of patient pathways, particularly for patients on hepatitis pathways, to ensure timely treatment and achievement of national targets.

The post holder will work closely with clinical and managerial staff, including the Assistant Service Manager, providing high standards of administrative support and actively tracking patient pathways. Responsibilities include monitoring patient progress, resolving day-to-day issues, escalating serious concerns, and ensuring effective communication and transfer of patient information between trusts and organisations.
