About the Role
We are recruiting to the newly formed Learning Disability Transitions/18-24 Team, offering high quality and timely transition support to young people and their carers in accordance with the Care Act and principles of Preparing for Adulthood. This is an exciting opportunity to join a dynamic team committed to empowering residents by drawing on their experiences, hobbies, communities and networks to build personalised care plans.

As a qualified Social Worker with at least two years post-qualifying experience, you will manage a caseload implementing short and long term interventions using a strengths-based approach to promote independence and wellbeing within the community. Your duties will include social care assessments, reviews, support planning, mental capacity assessments, Continuing Health Care decision support, and safeguarding enquiries as required.

Requirements

Experience
Minimum two years post-qualifying experience as a Social Worker with strong assessment skills and demonstrable experience delivering high quality interventions for residents, their families and carers.

Experience working within multi-disciplinary settings and building relationships with partners in the voluntary, community, faith and health sectors.

Sound understanding of Care Act interventions and significant knowledge of Strengths Based Practice, Mental Capacity, Court of Protection, Safeguarding Adults, and risk management based on current research and practice.

Qualifications
